By using the services offered by Coral LLC, the patient agrees to the following terms and conditions.

The patient certifies that he/she is of legal age and is fully authorized to enter into this agreement with Coral LLC.

The patient agrees to indemnify and hold harmless Coral LLC and its agents, employees, directors, officers, affiliates, and successors from any and all claims, damages, losses, expenses, and liabilities, including reasonable attorney's fees, arising out of or in connection with the use of Coral LLC's services.

The patient acknowledges that Coral LLC does not provide medical advice, diagnosis, or treatment, and that the services offered by Coral LLC are not a substitute for professional medical care.

The patient agrees to pay all fees associated with the services provided by Coral LLC, including any applicable taxes, in a timely manner. Coral LLC reserves the right to modify the fees at any time without prior notice to the patient.

The patient agrees to provide accurate and complete information to Coral LLC regarding his/her medical history, current health status, and any medications or supplements being taken. Coral LLC is not responsible for any harm that may result from inaccurate or incomplete information provided by the patient.

The patient acknowledges that Coral LLC retains the right to terminate this agreement and discontinue services at any time, without notice or cause.

This agreement shall be governed by and construed in accordance with the laws of the state of incorporation of Coral LLC. Any disputes arising from this agreement shall be resolved through binding arbitration in accordance with the rules of the American Arbitration Association.

This agreement constitutes the entire agreement between the patient and Coral LLC and supersedes all prior understandings, representations, and agreements between the parties.

The patient acknowledges that he/she has read and understands these terms and conditions and agrees to be bound by them.
